Preventive Medical Check-Ups

The preventive medical check-up is one of the most important tools for ensuring the safety and health of workers, preventing occupational diseases and preventing accidents at work.

The following individuals are eligible for preventive health examinations, performed in the Healthcare Room of Šeškinė Polyclinic:

  • drivers; 
  • employed individuals and persons seeking employment;
  • individuals wishing to acquire a civilian gun or to extend an existing permit;
    • train drivers and other staff employees involved in rail traffic;
    • inland waterway transport professionals and seafarers.

Pre-registration is available at the clinic reception, tel. (8 5) 250 2000 or online at e.poliklinika.lt.

Prophylactic examination is carried out in Room 128. Enquiries are accepted by phone (8 5) 250 2121. 

Health Examination of Drivers 

Persons wishing to obtain or renew a driving license must provide the following:  

  • a personal identity document;
  • a driver's license (for repetitive check-ups); 
  • a personal medical booklet (health passport, form No. 048 / a) if the person is employed as a driver on contractual basis;
  • a certificate prepared by a medical psychiatrist (form No. 027/a) if the person is not registered at Šeškinė Polyclinic;
  • a personal health extract issued by a family medical practitioner (form No. 027/a), if the person is not registered at Šeškinė Polyclinic;
  • a completed and signed Driver's Declaration on Honour (can also be completed on arrival).

Medical check-ups for employed individuals and persons seeking employment 

Persons seeking employment or already employed in conditions of potential occupational risk must submit the following: 

  • a personal identity document;
  • a personal medical booklet (a health passport, form 048/a) (for employed persons) or compulsory medical check-up certificate (form 047/a) (for persons seeking employment) that are to be completed and issued by the employer;
  • a personal health record (form 027/a) issued by a Family medical practitioner, if the person is not registered at Šeškinė Polyclinic.

Medical check-ups for individuals seeking to obtain a civilian gun or to extend an existing permit

Persons seeking to purchase a civilian gun or to extend an existing permit must provide the following:

  • a personal identity document;
  • a photo (3 x 4)
  • a character reference from workplace or ward;
  • a statement from a physician of addiction (form 049/a) for persons who have declared their place of residence in Vilnius: from the Centre of Addictive Disorders in Vilnius, at address Gerosios Vilties str. 3, reception tel. (8 5) 216 0014;
  • a personal health extract issued by a Family medical practitioner (form No. 027/a), if the person is not registered at Šeškinė Polyclinic;
  • a psychiatrist's opinion (form 049 / a).

IMPORTANT: persons who have been registered for less than 10 years at the Šeškinė Polyclinic must have a certificate issued by a physician - psychiatrist of the institution where they were previously treated.

Medical check-ups for train drivers and other staff involved in rail traffic

Train drivers and other staff involved in rail traffic must provide the following:

  • a personal identity document;
  • a personal medical booklet (a health passport, form 048/a) (for employed persons) or compulsory medical check-up certificate (form 047/a) (for persons seeking employment) that are to be completed and issued by the employer;
  • a personal health extract issued by a family medical practitioner (form No. 027/a), if the person is not registered at Šeškinė Polyclinic;
  • a certificate prepared by a medical psychiatrist (form No. 027/a) if the person is not registered at the Šeškinė Polyclinic.

 

Medical check-up for inland waterway transport professionals and seafarers

Inland waterway transport professionals and seafarers must provide the following:

  • a personal identity document;
  • a completed and signed Declaration of Honour (Medical Check-Up Questionnaire) for inland waterway transport specialists, seafarers and motor recreational craft and other motorboat pilots;
  • Individuals seeking employment must submit a medical certificate of compulsory medical check-up (form 047/a) to be completed and issued by the employer;
  • a personal health extract issued by a family medical practitioner (form No. 027/a), if the person is not registered at Šeškinė Polyclinic;
  • a certificate prepared by a medical psychiatrist (form No. 027/a) if the person is not registered at the Šeškinė Polyclinic.

Individuals who are registered at Šeškinė Polyclinic are issued with the required certificate by a psychiatrist at the Mental Health Centre.
